LPN – Clinical Support SpecialistPOSITION SUMMARY:The LPN Hospice Clinical Support Specialist is an office based position responsible for providing support and acting as a resource to clinicians in the field. You will assist in monitoring compliance, quality, and safe delivery of services by following a clinical process set forth by clinical management.PRIMARY DUTIES AND RESPONSIBILITIES:Operations: Serve as a resource for field staff, referral sources, physicians, patients and their families pertaining to clinical issues and/or concerns. Facilitate calls to and from Field staff; uses resources to resolve issues and reports anything of concern to manager. Assists with scheduling and clinical workflow. Assist in the evaluation of client needs for home health, appropriateness, and determining ability to accept referrals based on staff capacity. Supports agency quality and growth initiatives.Patient Care: Work in the field with staff and patients as needed. Participates in on-call per agency needs.Customer Service: Maintain positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted livings and families.Compliance: Obtains physician orders as needed.
