If you share this dedication and want a chance to make an even bigger impact, you’ll thrive here.About the Role:The Nurse Practitioner is responsible for conducting a face-to-face encounter and documenting the clinical findings supporting a life expectancy of 6 months or less.Role Responsibilities:Promote and practice the Sage Family of Companies philosophy and mission of “Providing the most compassionate, comprehensive, and qualified care to people with a life-limiting illness to serve them through their individual journey with dignity, respect, and integrity.”Working knowledge of the regulatory requirements per state licensure, and federal guidelines.Documents patient face to face (F2F) encounter in a timely manner immediately following the encounter.Demonstrate an understanding of the Centers for Medicare & Medicaid Services regulatory requirements specific to medical file documentation as it relates to the patients F2F encounter, and physical and functional decline.Properly utilizes prognostication tools such as, but not limited to: prospective payment system, formal assessment staging tool scores, Edmonton System Assessment Scale scores, and the New York Heart Association (NYHA) Classification.Documents utilizing quantifiable and comparative data from a full patient assessment.Advocate for healthcare changes which have the potential to improve quality of life and identify grief issues of patients and families by timely reporting of any concerns to a Clinical Support Manager.Collaborate with health care partners and referral sources to promote and sustain community relationships.Maintains proper certifications and licensing per state guidelines.Proficient in accessing and utilizing the organization’s electronic medical records.Performs other duties as assigned.Role Requirements:Master’s degree in nursing from an accredited program.Registered Nurse of an accredited school of nursing with a current license to practice professional nursing in the state in which we do business.Two (2) years of experience in hospital, medical outpatient clinic, hospice, home care or healthcare setting.Must have reliable transportation, proof of current automobile insurance and a valid driver’s license.Because those who care deserve more.Whether you are already in the care sector or looking to take your first step, we offer a unique opportunity.
