Our facility provides services for long-term care and short-term rehabilitation residents and is Medicare certified. The average daily census is 34 with approximately 10 skilled residents. We are hiring a Resident Assessment Coordinator for our Skilled Nursing team. In this role you will ensure the accurate completion of MDS assessments and care plans as well as provide guidance to the interdisciplinary team in the delivery of a holistic person-centered, individualized and clinically competent care.
